Case Management is one of our services offered in the community of Louisville and surrounding counties. A Target Case Manager is professionally trained and certified by the commonwealth of Kentucky. Their assistance to our clients includes, but is not limited to:

Connecting clients with resources they need to meet basic needs. 

Helping our clients with the skills they need to be self-sufficient.

Acting as a coordinator between treatment providers in situations where many professionals and other members of family are involved.
